Time Blindness: Why an Hour Feels Like Twenty Minutes with ADHD

A person with ADHD is not late because they do not respect other people's time. And they do not postpone a report because they do not care about the outcome. In both cases, the same malfunction is at work: time is perceived inaccurately. It is not "poorly planned" — it is perceived, at the same level as sound volume or distance to an object. An hour can feel like twenty minutes, and "in two weeks" can feel like "never." This malfunction is described by the term - time blindness, and it explains a significant part of what appears to be irresponsibility from the outside.

This is not about punctuality.

Time blindness is not included in the formal diagnostic criteria for ADHD, but it has been described in the literature for a long time and consistently—largely thanks to the work of Russell Barkley, who viewed the deficit in time regulation as one of the key core manifestations, rather than a side inconvenience. It manifests in several recognizable forms.

The first is a systematic underestimation of how long things will take. "This will take ten minutes" turns into forty, and it happens every time, despite all previous experience. Experience accumulates poorly here precisely because the estimation is not based on memories of past attempts, but on a feeling in the moment.

The second is the disappearance of time within a task. During hyperfocus, three hours pass like one: the internal timer doesn’t so much get off track as it simply stops being felt. Hence, missed meetings for a person who was diligently working at that moment.

The third is "just five more minutes," stretching to an hour. An interval that is not marked by anything external is almost not measured from the inside.

In experimental tasks, this is seen directly. When people are asked to reproduce or estimate an interval of a few seconds, the group with ADHD has both lower accuracy and noticeably greater variability in results—meta-analyses consistently document this effect, not only in children but also in adults. This means it is not about habit or character, but about the functioning of a specific mechanism.

Where in the brain are the clocks located?

There is no separate "time organ" in the brain. The assessment of duration is gathered from the work of several systems, and different ranges are serviced differently.

Milliseconds are the domain of the cerebellum: it is responsible for the timing of movements and speech, ensuring that steps are rhythmic and syllables do not overlap. This level is less affected in ADHD and is almost unnoticeable in daily life.

Seconds and minutes are the exact range where "five minutes" and "half an hour" live, and they are serviced by the connection between the striatum and the prefrontal cortex. Simplified: the striatum works like a counter, and the prefrontal cortex holds the standard against which this count is compared. And dopamine directly affects the speed of counting.

This is not a metaphor: in studies of interval timing, substances that enhance dopamine transmission speed up the "internal metronome," while those that block it slow it down. Subjectively accelerated internal clocks mean that the actual interval seems longer, while slowed ones mean it passes unnoticed. In ADHD, dopamine transmission in striatal circuits works differently, and this is exactly the same neurotransmitter responsible for motivation and action initiation. This is added to working memory. To feel how much time has passed, you need to hold the starting point in your head — and with weak working memory, that gets lost first. Without a reference point, you have nothing to subtract from.

The "now or never" effect

The second half of the problem concerns not the past but the future. In psychology, it is described through temporal discounting: the further away a reward is, the less its subjective value. This is true for everyone — a thousand rubles now is more pleasant than a thousand in a year. But for people with ADHD, the devaluation curve is steeper: distant rewards are devalued faster and more strongly. Meta-analyses record this as one of the most reproducible behavioral signs.

Barkley called it temporal myopia: the horizon within which the future feels like the present, compressed. Beyond it, events are not so much forgotten — they don’t gain enough weight to compete with anything happening right now. A deadline in two weeks is known, written down, confirmed in the calendar, and yet subjectively does not exist.

Here’s a familiar scenario with the night before a deadline. It looks like procrastination, but the mechanics are different: the task has finally entered the field of vision, along with it came stress activation, and with that activation came the level of norepinephrine and dopamine at which the brain can kick into gear. Working at the last minute is not because a person loves adrenaline, but because the task had no subjective weight until the very last moment.

Why a reminder a week in advance doesn't work

From the two described mechanisms, an unpleasant conclusion follows for most reminder systems. The notification states a fact: an event on such and such a date. But the problem is not in the lack of knowledge about the fact. A person already knows about the deadline. The problem is that the fact does not convert into a sense of urgency, and without that feeling, nothing gets started.

A reminder a week in advance falls exactly into the zone where the task doesn't subjectively exist yet. It wastes its one activation: it doesn't cause anxiety, it doesn't trigger action, and by the time the deadline finally approaches, it's already spent. Worse, a series of notifications that you can't respond to right now teaches the brain to ignore notifications altogether — and then even what comes on time gets missed.

What helps in practice

Everything that works here is arranged the same way: it doesn't train your sense of time, but compensates for it with external supports. This is prosthetics, and you should treat it exactly like that.

  1. Shrink the time horizon. Not "submit the project by the twentieth," but "today by noon — one specific piece." The point is not in breaking the task down per se, but in moving the deadline into the zone where it is felt. A distant date won't feel more significant just because you think about it more often — but this evening is significant in itself.
  2. Making time visible. The number 14:32 requires calculation: how much has passed, how much is left. A decreasing colored sector on a timer or an hourglass is perceived directly, without arithmetic. The difference is more significant than it seems: it relieves the load on the very system that is not functioning well.
  3. Measure, not evaluate. For two weeks, record the actual duration of routine tasks—getting ready at home, travel, sorting mail. Then use a table instead of intuition. Intuition won't change, but it won't have to make decisions anymore.
  4. Break long intervals with external breaks. Interval work with a timer helps not with motivation, but by turning a formless hour into measurable units, each of which has a clear end.
  5. Set the alarm for the exit, not for the entrance. With hyperfocus, the problem isn't starting, but noticing when it's time to stop. The "stop" signal is more useful than the "start" signal.
  6. Tie events to anchors, not to times. "After lunch," "as soon as I get back" work more reliably than "at 3:00 PM" because they rely on sequence rather than duration—and with sequence, the brain with ADHD does better.

Where the tricks end

None of these methods restore the sense of time. Time blindness doesn't get trained back to normal through exercises, and promises to the contrary should be read skeptically. A realistic goal is to reduce the number of situations where a breakdown leads to consequences: a missed meeting, a missed deadline, a night before submission.
